Case Manager - CPP

Provides case management services to juveniles admitted to the new Community Placement Program (CPP), still wards of the Department of Juvenile Justice (DJJ), at the Merrimac Juvenile Detention Center. The employee must continually assess the child/adolescentís functioning level interms of risks and needs and make the appropriate referrals for sevices. The employee will apply various case management principles and practices to children/adolescents, and families who experience a wide range of problems including mental illness, intellectual disabilities and substance use disorders, criminal justice issues, medical problems, school problems, problems in vocational functioning and various other social problems. Services include assessment, linkage to resources, coordination of a range of service providers, supportive counseling, advocacy, monitoring, implementing and monitoring case plans, and completing appropriate documentation as required by the Center and the Department of Juvenile justice. Starting salary range $43,657. 


A baccalaureate degree in behavior science or related field and four years of professional experience working with adolescents, one year of which must have been in a residential facility for children and at least one year providing case management services;

Requires an acceptable background investigation. Individual cannot be listed with any state as having a founded child abuse complaint, cannot use illegal drugs on or off the job. Must maintain a current tuberculin test, and all certifications and training as required by policy and applicable standards.
